LEAP sees the importance of housing as a “setting and system” where health has occurred, does occur, and will occur. We strive to see the setting and system cultivated to protect, build, and maintain positive Social Determinants of Health (SDOH).

Falcons Landing Health Recommendations Report

In a partnership with the Blue Cross of Idaho Foundation for Health and the Western Idaho Community Health Collaborative (WICHC), LEAP will focus on health at Falcons Landing through awareness and prioritization of social determinants of health. Falcons Landing is LEAP’s first health-focused housing project.

WICHC reviewed a number of community health data resources and determined the following things are the top three health priorities in the community of Mountain Home:

  • Mental & Behavioral Health

  • Chronic Disease

  • Community Connection

WICHC also provided several recommended steps for LEAP to implement at Falcons Landing, including:

  • Establishing a resident board

  • Employing a community coordinator

  • Partnering with community health workers

  • Creating a community pantry

  • Integrating culturally inclusive colors and designs for the Latinx community
